General Participation Waiver:
Notice: The City of Cudahy is not responsible for injuries sustained from use of City facilities/fields. Photographers employed by the City may visit our programs and take photos of participants for promotional purposes in City publications.
General Release, Waiver and Assumption of Risk: I, the undersigned, understand the potential risk(s) involved in participating in this physical activity. Dangers could include, but are not limited to cuts, broken bones and other debilitating or permanent physical and emotional injury, damage to personal property, or death.
Participation in the City of Cudahy’s Youth Basketball League exposes participants to the risk of personal injury, death or property damage. I hereby acknowledge that they are voluntarily participating in this class and I agree to assume any such risks.
I hereby release, discharge, and agree not to sue the City of Cudahy for any injury, death or damage to or loss of personal property arising out of, or in connection with, my participation in the program from whatever cause, including the active or passive negligence of The City of Cudahy or any other participants in the class.
In consideration for being permitted to participate in the program, I hereby agree, for myself, my heirs, administrators, executors and assigns that I shall indemnify and hold harmless the City of Cudahy from any and all claims, demands actions or suits arising out of or in connection with enrolling a participant in the class.
I HAVE CAREFULLY READ THIS RELEASE, HOLD HARMLESS AND AGREE NOT TO SUE AND FULLY UNDERSTAND ITS CONTEXTS. I AM AWARE THAT IT IS A FULL RELEASE OF ALL LIABILTY AND SIGN IT ON MY OWN FREE WILL.
